These simple fitness tests revealed which older adults had the lowest death risk

A recent study published in JAMA Network Open examined the relationship between objectively measured physical fitness and all-cause mortality among adults aged 65 years and older. The findings suggest that better physical fitness may be a strong indicator of longer survival and could complement conventional health assessments in older adults.

Researchers analyzed data from 13,423 participants in Taiwan, linking national fitness and health records. Participants underwent assessments of cardiorespiratory fitness, muscular strength, flexibility, balance, and agility. They were followed for approximately seven years, with mortality outcomes tracked through the end of 2022.

Older adults with higher fitness levels consistently showed lower mortality risk. Those in the highest composite fitness group had a 61% lower adjusted risk of death compared with those in the lowest group. Balance and agility showed the strongest individual associations with survival, followed by lower-body strength and cardiorespiratory fitness. The greatest differences in mortality were observed among participants with the poorest fitness levels.

The findings highlight the potential importance of incorporating simple, objective measures of balance, mobility, strength, and aerobic fitness into geriatric care. Identifying adults with low physical performance may help clinicians recognize higher-risk individuals and support targeted interventions aimed at maintaining function and promoting healthy aging.

The study was observational and measured fitness at a single time point, so it cannot establish causation. Additional research using repeated fitness assessments and more diverse populations is needed to determine whether these measures can improve mortality prediction and guide healthy-aging interventions.
